For screening I need: a photo of a government-issued ID (driver’s license, passport, etc.) with name, photo, DOB, and expiration date visible. Your real phone number attached to your name (no Google voice). We do not need to communicate via phone, email is fine. It is just required as part of screening. Generally, this is enough to perform my screening, but I reserve the right to ask for more information if needed.
